How to Write a CAPA for Customer Complaints
CAPA stands for Corrective and Preventive Action. It's how you make sure a customer complaint is fixed and doesn't happen again.
Step 1: Log it the same day
Record the complaint number, date, customer, what exactly went wrong, severity and who owns it. Acknowledge the customer within 24 hours, even if you don't have answers yet.
Step 2: Contain and correct
Stop the problem from spreading and fix what the customer has: sort the lot, rework, replace, send a credit note. This is the correction.
Step 3: Find the root cause
Ask "why?" until you reach something you can change in your process. Example:
1. Why was the shaft out of tolerance? The cutting insert was worn.
2. Why wasn't the wear noticed? Nobody checked during the shift.
3. Why not? The checklist only had a start-of-shift check.
Root cause: no mid-shift tool check. "Operator mistake" is rarely a root cause; ask why the process allowed it.
Step 4: Corrective action
Change the process so the cause is removed: add the mid-shift tool check to the checklist, train operators, and update the control plan. This is the corrective action. Preventive action applies the same learning to similar machines or products before they fail.
Step 5: Set deadlines by severity
A common rule: critical complaints closed within 7 days, major within 15, minor within 30. Track overdue items every week.
Step 6: Verify effectiveness
After a few weeks or the next few deliveries, check: has the problem come back? If yes, the root cause was wrong; reopen and dig deeper.
Step 7: Close the loop with the customer
Send a short report (many customers ask for an 8D): problem, containment, root cause, corrective action and evidence. Customers forgive a problem far more easily than silence.
Watch for repeats
If the same customer raises the same type of complaint again, treat it as a priority. It means the corrective action didn't work.
Our Customer Complaint & CAPA Register separates correction from corrective action, sets targets by severity, tracks effectiveness and flags overdue and repeat complaints.
